The certificate includes the following information:
Either a confirmation that the person is alive or a statement that it could not be confirmed whether they are alive or not, name, date of birth, municipality of residence and permanent address.
In addition, you can choose whether previous names will be shown on the certificate. You can choose to replace the date of birth with personal identity code. If the person concerned has a legal guardian or a legal guardianship authorisation, this will also be shown on the certificate.
